Use this form to document radioactive material use and disposal.

Track the estimated amount of radioactivity in each waste bag or container by recording the date, activity, and isotope.

Use a separate inventory control sheet for each item that contains radioactive materials.

1. Complete the lab information and item information when you receive the item or material.
2. Update the use and disposal log each time you use or dispose of an item. Make sure the amounts recorded for each waste stream add up to the total amount used (you can estimate amounts to be within 25%). Always circle or mark units consistently.
